- ●Those with "ineligible" residence status cannot apply.
- ●If you are applying for an installment purchase contract or individual credit purchase brokerage contract, you will need the following confirmation documents.
- ●If your current address is different from the address on the confirmation document, you will need to provide supporting documents such as a residence certificate or a utility bill with your current address written on it (issued within the last 3 months).
- ●If your period of stay does not meet the installment contract period or our standards, you will not be able to enter into an installment purchase contract or individual credit purchase brokerage contract.
- ●If you are cancelling your contract when you return to your home country and are therefore unable to bring your residence card with you because you have already returned it, your foreign-issued passport will be accepted.

- ●The health insurance card must state your name, date of birth, and address, and if it has an expiration date, it must still be valid.
- ●In the case of National Health Insurance Cards, the city, town or village where the card is issued must be the same as the city, town or village where the applicant's address is stated.
- ●Certificates of eligibility will not be accepted.
- ●The parental consent form must be completed and stamped by the parent or guardian themselves.
- ●In principle, if you are between the ages of 12 and 16 and apply for an individual credit purchase brokerage contract (installment sales contract), you must make payment using a bank account or credit card in your legal guardian's name.

- ●If the contract holder is a minor who is admitted to a child welfare institution and the person acting as parental authority is an official of the institution, the following identification documents are required.

If a representative visits the store
If it is difficult for the contract holder (or representative or person in charge in the case of a corporation) to visit the branch to carry out various procedures, a power of attorney is required for the procedure.
Please also submit identification documents for the representative (including family identification documents if the line is in an individual's name).

Family Verification Documents
Family verification documents are documents that prove your relationship to the policyholder.
Documents verifying that you have the same surname or address,orA single document that can prove your relationship with the contract holderis applicable.
Please bring one of the following:
- ●Identification documents to verify that you have the same surname or address
- ●Evidence of your relationship with the contract holder
(A "Certificate of Resident Record," "Health Insurance Card," "Remote Area Insurance Card," "Resident Record," "Certified Copy (Extract) of Family Register," etc. that lists the names of both the policyholder and the agent. However, "Certificate of Resident Record," "Resident Record," and "Certified Copy (Extract) of Family Register" must be issued within the last three months.)
