For family members living together
Please prepare the contract holder's identification document that proves that you have the same address.

For family members living apart
One of the following documents is required:

If the parties have the same surname, only "1. Identification document of the contract holder" will be accepted.
In the case of different surnames, we will accept ``2. Document verifying that you are a family member'' or ``3. Health insurance card with all family members' names listed on one page.''
