If you are switching from Docomo, au, Rakuten Mobile, or other MVNOs other than SoftBank, you will need to have an MNP reservation number issued. Please request this from the MNP reservation desk of your current mobile phone company before applying.
- *Online applications require a 10 day validity period.
- *When transferring your number from SoftBank, you do not need to issue an MNP reservation number.
- *For some models, we do not accept transfers from MVNOs that use SoftBank lines.
- * Depending on the carrier from which you are transferring, you may be able to complete the procedure at the online store using a method that does not require an MNP reservation number (one-stop).

If the contract holder and the credit card holder (payer) are different, the following documents are required.

- *Please prepare identification documents that can verify the user's age, or a student ID or student handbook.
- *If you purchase only accessories, you do not need to send identification documents.
- *If you are a foreign national and would like to choose installment payment when purchasing a device, your period of stay must be within the installment payment period. Therefore, please prepare one of the above identification documents (8), (9), or (10). Please check "Uploading identification documents".

When applying for a secondary line for the Family Discount Service, you will need a Family Discount Service Agreement and identification documents that prove that the contract holder is the primary line holder and a family member. Please see above for details on identification documents.
- *Definition of family: Family members (blood ties or marriage) or living in the same address (sharing the same household) can be confirmed.
- *If the name of the secondary line is the same as that of the main line, it is not necessary to submit a Family Discount Service Agreement or family verification documents.

Documented verification of same-sex partnership
These include certificates, copies of affidavits, and receipts issued by each local government, which include the following conditions:
- The subject is listed on the same certificate
- The name of the person in question matches the information in the application
- If your local government issues a receipt, you will also need one
- *Names vary depending on the local government.
Verification by health insurance card
The conditions for a health insurance card to be used as family verification document are as follows:
- The names of your family members must be listed on your insurance card
- The date of issue must be stated
- If there is an expiration date, it must be within the expiration date.
- ※ Regarding confirmation using the "Remote Area Insurance Card"
- If all family members joining are listed, one form will be accepted.
- If not all family members are listed, please provide each family member's remote health insurance card and identification document.

Also, be sure to upload the back side of the document as an additional document.
